The annual open enrollment period to enroll in health insurance usually falls around November 1st to December 15th. Outside of these dates, only eligible individuals can enroll in a health insurance policy if they qualify for special enrollment period (SEP). If you qualify for special enrollment, you usually have up to 60 days following the qualifying event to enroll in a plan and you are also required to provide proof.

· New dependent living with you, such as a grandparent
· Marriage
· Death in the family
· Loss of dependent status
· Divorce
· Retiring spouse:
Ø Change in employment status
Ø Loss of individual health coverage or a plan you bought yourself
Ø Change in residence
· Moving out of state and, in some cases, moving to a different county in your state
· A student moving to or from a place he or she attends school
· A seasonal worker moving to or from the place he or she works
· Moving to or from a shelter:
Ø Leaving incarceration (jail or prison)
Ø New U.S. citizenship
Ø Loss of eligibility for Medicare or CHIP
Ø Return from military service
