The Department of Labor (DOL) released new Family and Medical Leave Act (FMLA) forms, On July 16, 2020. These new forms can be used to comply with many technical nuances of FMLA, but, like prior versions, are completely voluntary.

The revisions to the forms were made in an effort to make them easier to understand, eliminate unnecessary questions, and reduce the administrative burden on both providers and employers. While employers are still free to use the old forms, they are cautioned not to ask for any information other than what is specified in FMLA regulations
