The Affiliated Physicians and Employers Heath Plan is committed to the health and well-being of its members. This year’s wellness rewards program is our way of rewarding you and your employees who are enrolled in the Health Plan for putting their health first. Below is a summary of how the program works:
EMPLOYER PARTICIPATION
If 80% of your employees (enrolled in the health plan) register for the Wellness Program, and 80% of those registered complete the program by December 31, 2018, you will be eligible for a health care fee credit of 1% of your paid health care fees 2.
Example: You have 12 employees, 2 waived coverage, 10 employees enrolled in the health plan. In order to be eligible, 8 of those 10 enrolled employees must register for the Wellness Program. For you to receive your 1% reward, 7 of those 8 employees who registered must successfully complete their Health Actions form by December 31, 2018.
EMPLOYEE PARTICIPATION
Enrolled employees age 18+ are eligible to participate and can earn a $50 cash reward for completing an online health assessment and the actions listed on the Health Actions form by December 31, 2018 1:
